The first visit to our office is designed to get you better acquainted with our practice and our doctors and staff. Prior to scheduling a required appointment with our nutritionist and one of our bariatric surgeons, you must attend one of our Bariatric Seminars. Click Here for information regarding mandatory attendance at one of our Bariatric Seminars, seminar dates and times as well as seminar registration information.  

Unfortunately due to the insurance climate, we do not accept medicare without secondary insurance (this does not include medicaid)  or any medicaid insurance beginning January 1, 2010 for VNUS.  We will not be accepting medicare/medicaid for Bariatrics; we will continue to care for our current patient population. 

Below you will find our New Patient Information & New Patient Registration packets that you can review, download and print from home. The New Patient Registration packet must be completed and returned to our office prior to scheduling your appointment. Once you have completed the required forms you can mail, fax or drop off at our office. Once we review your forms, we will contact you to schedule your appointment. These forms are also available at our mandatory bariatric seminars. 

*Again, attendance at one of our Bariatric Seminars is mandatory prior to completing the New Patient Bariatric Surgery Registration Packet and scheduling your appointment.
